Dreams Come True works with many wonderful referral partners and physicians who help us identify brave children currently battling life-threatening medical conditions in our area. Children who live and/or are treated in Northeast Florida are eligible if they meet the following criteria:
Eligibility:
- Live and/or treated in Northeast Florida
- Between the ages of 2 ½ and 18
- Diagnosed with a life-threatening medical condition
- Referred by a physician
- Medically approved to participate
- Has not received a dream/wish or other services from another dream-granting organization
How To Refer a Child
Parents or guardians of a child may refer them through our online referral form.
